Jeff, let me explain my diagram.
The grey text refers to the pin setup that Gamelver did in his initial, (slightly confusing) guide. The red pins refer to the actually pin setup as printed on the Gamecube motherboard. Ground should be obvious on the board, since it's about 90% of the gamecube motherboard. As long as you ground the board, it doesn't need to be included in the diagram, but if I remember correctly, Pin 1 to 1 should ground.
Also, my bad about the extra holes, It should still be the same with the last few pins being at the end..
