The Oddysey perhaps? they are about 4" by 2" and about 5mm thick.

It is technically a home console.

The PC engine Hu Cards are the second smallest if i remember correctly the SMS Sega card is the smallest (it's the same size as the Hu Cards but its about 1cm smaller in height altho im not sure
