Does anyone know how to setup a phpBB forum?

If you're going to be hosting it on your own site, there should be information at www.phpbb.com. If you are looking to host the site somewhere else, www.ezforum.net is the host I use. It's very simple to set up.

www.ezforum.org is where I have my crappy spinoff forum hosted.

www.phpbbforfree.com is fast, but there's ads EVERYWHERE!!!

Your best bet is installing phpbb to your own server, but it's not the easiest.
