Honestly, everything is gone except for the casing... I threw it all out about 2 weeks ago. I'm using the plastic to seperate everything in my project... ... otherwise, i would've just given it to you free. If I do see something else lying around, I'll hit you up.
Try looking at the pics above. It's all right there! Counting from the top down, look at them in this order: #8 (input board - everything you asked about starts here), 7 and 6 follow the cables where they lead, and finally #3 shows where the last one ends up. You'll have to follow the metal circuit tracks on the boards to know exactly which wire does what, but this stuff is so simple I really don't know why anyone would ask in the first place.
