Okay, here's my first "death" in a video game.
I was watching my mom play SMB 1 on the NES. I could remember that there was pressing of "B" to fire fireball.
I grew up a little and tried to play it for the first time. There was this first mushroom-shaped enemy (Goomba) then I tried pressing B repeatedly to fire a fireball but nope. No fireball came out and it was too late for Mario.

It was in Super Mario Land for the Gameboy. I got to the second level, and didn't make the jump.

I can't know for sure, but I bet it was in a Scott Adams text based adventure when I tried to move around in a dark cave before lighting my lantern and ended up tumbling into a ravine. I can reenact it here.

My first video game death was either getting killed by an alien in super metroid or not understandind how to jump in super mario world
or maybe even earlier getting bitten to death by the dogs in metal gear for the nes
I still play these games all the time
