Hm...

1. To get anywhere, you will have to put yourself in uncomfortable situations. If it's a challenge, then GOOD. Hard is GOOD.

2. Pay attention to your physical appearance. Don't stress over it, but do make an effort to maintain personal hygiene, wear different clothes, stand up straight. If you look hunched-over and miserable, noone is going out of their way to talk to you.

3. Make yourself invitable to conversation. When you talk, enunciate (say words clearly so you're not mumbling.)

4. When you meet someone, try to have at least a 30sec conversation. Say, you're going to RadioShack to buy parts. Ask the salesman something like "How's the business lately?" I was buying a Hummer DTV game and the guy was intrigued when I told him the game was based on a Commodore 64.

5. Break the ice with a wisecrack or something. "That kid over there should join the Olympics with the size of that backpack." Ok, it's cheesy, but humor will make the rest of the conversation easier.

Keep your head up, stand straight, and talk clearly. You don't have to be loud, but don't make people strain to listen to you, because they won't.

You'll have to make the preemptive strike. Practice, you will be uncomfortable, I guarantee you. But the only way to get better is to slog through the awkward moments.

Like go to a social place or gathering and don't go with friends. I can understand how unnerving that is, I'm still quite anti-social.

The only way to build those social skills are to get out in society and start communicating. I kind of had help because of college, but you just need to get out there and talk to people you don't know. Build that skill in finding a common interest with a stranger, once you get that going talking with people in general will be a simpler act.
This is a very good idea

similar to my solution however mine is probably a little imcompatible for your situation seeing as it involves alcohol

I will admit getting friends isnt effortless for anyone, although some do have a harder time than others.

go to gigs of bands you like, start up conversations with people. If people talk to you then continue the conversation rather than one word answers. Just try and let go of your fear, if you have nothing, then theres nothing to lose so if you at first you dont succeed then theres no need to worry.

At the end of the dya just try and think "this doesnt really matter if they like me or not" then you'll lose the fear, loosen up and it'll be easier for people to talk to you
