I don't have really large hands either, but I still prefer the old big handhelds (Atari Lynx, Sega GameGear) over the small modern ones (gba, gba sp, gb micro, psp, nds). For some reason the big ones cause a lot less discomfort when using them for a prolonged period of time.

If there aren't enough buttons, you can always add one, that's what modding is all about anyway, modifying the stuff :P
There are pins on most controllers that aren't connected, so it would be to connect a pin that was previously unconnected.
