Well, the GC runs fine, and everything is normal, right?
Well, what happens during gameplay is, when "the lid" is opened, the disc stops, but the gamecube doesn't know. It keeps playing until it needs to read the disc again and then it gives me the error message.
And then when its closed right after its opened, the disc stops, and it plays until it needs to load, I get the error message, and then it starts to load again.
By the way, the error message basically tells me something is wrong and I need to look in the manual(wtf, theres nothing in the manual) and that I have to turn it off.
Uhh, help?

Unbridged: lid is open

To elaborate, the only reason this is a problem is:
A. I cannot use the Action Replay
B. I cannot play any multi disc games
C. I cannot load anything with my qoob sx.
The real problem is C.
I cant figure this out at all... it will run normal games fine, but, I can't ever open the lid after boot.

Apparently, the gc has two sensor to identify that the cover is open.
When the wires are unbridged, the drive automaticly stops. But if you give pin 12 on the gc mobo 5v, it wont know the difference. And then later when it needs to read it will go into OMGWTF mode.
Turns out I had the 5v pin and the dicover pin bridged. whoops.

if you are u using some mod chip like qoob pro/sx
I had a similar isue using the qoob pro mod chip, to fix, I connected the lid connection of gamecube like before the mod , and connecting the chip lid switched cables to itself, it worked for me and other ppl.

seems you have qoob sx installed, so I think this will work for uC. I cannot load anything with my qoob sx.
